I will provide physical and emotional care for this animal, including, love, companionship, food, water, appropriate shelter, humane care, exercise, and other interactive play including toys. I understand that appropriate shelter for a dog means that the dog will reside in the family home and not be housed or chained out-of-doors. I understand that any cat shall be kept indoors.

I will provide appropriate veterinary care and in particular recommended vaccinations including Rabies and follow-up boosters to keep this animal in good health

I agree that the animal will be micro-chipped (a form of permanent identification) by ACN.

I agree that ACN shall not refund to me the adoption fee should the adoption not work out. I understand that an exchange for another ACN animal can be made within two (2) weeks of this adoption date, subject to approval by ACN.

I agree that ACN is not liable for the temperament, health, or mental disposition of the animal I have adopted. I accept full responsibility for this animal, and do hereby release, discharge, and hold ACN harmless from any injury and/or damage to persons or property caused by this animal and from any action, suits, claims, or demands that arise as a result of the behavior or conduct of the adopted animal.

I agree to comply with all the Laws and Ordinances relating to the ownership, licensing, and treatment of an animal that are in force and effect in the political subdivision in which I live.

I agree that, should I become unable to care for this animal for any reason or should I believe that I cannot care for this animal in the manner that I have heretofore agreed to, I shall immediately contact ACN. I specifically agree that I shall contact ACN prior to arranging for other care of the animal or making any other disposition of the animal such as euthanasia (unless the animal is suffering from mortal and painful injuries and such course has been recommended by a veterinarian). I acknowledge that I have read and understand ACN's Return Policy that is stated on Exhibit A to this agreement.

I agree that I shall contact ACN immediately should this adopted animal go missing.

I agree that ACN shall have the right to do home inspections and/or to make follow-up phone calls to me in its efforts to assure that the terms of this contract are being adhered to and to ensure the adopted animal's welfare.

I acknowledge and agree that the information that I have provided to ACN in its Screening Form, in this Adoption Agreement, and in any other informational form such as Exhibit A attached hereto is true and correct. I understand and agree that my failure to provide truthful and accurate information shall constitute a material breach of this Adoption Agreement and that ACN shall have the absolute right to the return of the animal being adopted.

I agree that, should it become necessary for ACN to hire an attorney and to pay any costs related to the enforcement of the provisions of this Adoption Agreement, I shall be liable for any such attorney's fees and costs, said fees and costs to be no less than $ 400.00.

I acknowledge and agree that I have read and understand this Adoption Agreement and each of its terms and conditions.

Cats will have already been tested for FIV and feline leukemia.
